Nevada Care Connection (NVCC) is our states established network of aging and disability resource centers, and it has been administered by Access to Healthcare Network (AHN) to serve as a single point of entry of assistance with information and resource navigation of long-term support services for older adults, people with disabilities (of any age), family members, and care providers for the following 12 Northern Nevada counties: Washoe, Carson City, Churchill, Douglas, Elko, Eureka, Humboldt, Lander, Mineral, Pershing, Storey, and White Pine.

NVCC helps individuals of all ages and income levels obtain the services and support they need by establishing one-on-one assistance through a single organization. This one-stop-shop works to promote independent living by providing access to needed services and reducing the stress of navigating benefits.

Who is Nevada Care Connection for?

  • Individuals 60 years of age and above

  • Individuals with a disability regardless of age

  • Individuals who are Deaf, Hard of Hearing, or Speech Impaired

  • Veterans

  • Family members/caregivers or care partners

What does it provide?

  • Resource & service navigation: trained and certified staff to assess your needs over the phone or in-person, help you understand all available options for assistance, and empower you to make informed decisions

  • Community resource assistance including: food, housing, and energy assistance

  • Pre-determination of eligibility for public programs which include but not limited to

      • Medicaid Application
      • SNAP (food stamps) application
      • Medicare Savings Program applications
      • Home and Community Based Waiver programs
  • Help Connecting individuals to services and resources they need to meet their long-term goals such as:

      • Caregiver support
      • Supportive housing
      • Independent living services
      • Home modifications
      • Financial planning
      • Short- and long-term planning
  • Case Management: for individuals who have a difficulty maintaining services, have cognitive or functional impairment such as dementia, mental health or traumatic brain injuries that hinder abilities to maintain support services.

    While Resource and Service Navigation is offered to every individual accessing Nevada Care Connection, case management services are targeted to individuals who have a higher level of need to monitor and follow-up on services specified in the individual’s plan, ensuring the services are being provided in accordance with the individual’s plan

  • Serving rural Veterans directed care programs that give Veterans the opportunity to receive the home and community-based services they need in a consumer-directed way.

Save Money on Medicare Costs

AHN’s Aging & Disability Resource Center can assist individuals in applying for the Medicare Savings Program, which if approved, can result in individuals saving up to $144.60 per month on Medicare Part B.

AHN’s Aging & Disability Resource Center can also assist individuals in applying for the Extra-Help low-income subsidy which providers up to $26.50 per month in Medicare Part D funding and provides additional discounts on prescription drugs.

Individuals approved for both the Medicare Savings Program and the Extra Help Prescription Program receive an average of $5,000 in Medicare-related savings per year.
