Northern Nevada Care Connection Resource Centers
Nevada Care Connection Resource Centers (NVCC) serve as single points of entry into the long-term services and supports (LTSS) system for older adults, people with disabilities, caregivers, veterans, and families. NVCC’s are “no wrong door” systems for individuals seeking assistance related to being aged and or disabled. Access to Healthcare Network administers the NVCC for Washoe, Carson, Mineral, Douglas, Elko, Eureka, Lander, Humboldt, White Pine, Churchill, Pershing, and Storey counties.
The vision behind NVCC is to establish, in every community, highly visible and trusted sources of information where individuals can turn for the full range of long-term support options and smooth access to public long-term support programs and benefits. In addition, NVCC ensures that Nevadans with functional limitations and the family members that support them have timely access to correct information and quality services that promote choice, dignity, and independence.
NVCC provides unbiased, reliable information and counseling to individuals with all levels of income. They assist a wide range of individuals, including family caregivers, in obtaining long-term supports and services in the most desirable and appropriate setting. Since NVCC’s do not limit their services to low-income individuals, they can help families of all income levels get the help they need as well as utilize their resources more wisely.

Options Counseling
Trained Options Counselors provide individualized person-centered decision support to consumers, family members and/or significant others in the home, office, or over the phone depending on consumer preference.
Action plans are developed to address long term services and supports needs that align with consumer preferences, strengths, values, and needs.
Follow-up is provided to ensure consumer needs are addressed.
Person-centered transition support is provided via formal links between health care providers and NVCC services and follow-up with nursing home residents who request information on community options.
Available Services
Nevada Care Connection Resource Centers provide one on one assistance to older adults, people with disabilities, caregivers and families. The Resource Centers offer three types of services:
Resource & Service Navigation – a person-centered process that helps individuals and their families explore care options in Nevada
Caregiver Support – helping Nevada’s family caregivers connect to a toolbox of services and empowering them in their caregiver role
Case Management – offered to seniors and individuals with disabilities who are in need of long term support services
Veterans Services – connecting to individuals who have served in the military explore benefits available through the VA, the state, and informal supports
Medicare Improvements for Patients and Providers Act
AHN also helps Nevadans apply for the “Extra Help” low-income subsidy, which helps pay for Medicare Part D up to $20.56 per month and additionally provides discounted prescription prices.
A Nevadan approved for both of these subsidies receives an average of $5,000 in savings per year.
