Diabetes is among the most common, costly, and serious chronic diseases affecting our communities. In Nevada alone, more than 280,000 adults (12.4% of the adult population) live with diabetes and face its devastating consequences every day. When left untreated, diabetes can lead to other potential health complications, such as heart disease, stroke, kidney damage, and nerve damage.

Learning to manage your diabetes may seem overwhelming at times, but Access to Healthcare Network is here to help. Whether you are newly diagnosed or have been living with diabetes for many years, our Diabetes Self-Management Education (DSME) program can assist you in managing your diabetes, achieving your health goals, and improving your overall quality of life.

The Access Diabetes Self-Management Education program is a six-week workshop available for Nevada residents who are living with type 1, type 2, or gestational diabetes. Under the guidance of a Registered Dietitian and trained Health Educators, workshop participants learn the skills needed to manage blood sugar levels through diet, exercise, and healthy lifestyle modifications.

Included in the Access Diabetes Self-Management Education Program

  • One-on-one consultation and comprehensive assessment with a Registered Dietitian

  • Personalized education plan focused on participant identified goals

  • Six group education classes, including nutrition education and cooking demonstrations

  • Follow-up assessment and access to additional resources and support as needed

  • Diabetes overview, including disease process and available treatment options

  • Healthy eating and nutrition, including cooking demonstrations and recipe tasting

  • Exercise and physical activity

  • Medication usage and safety

  • Reducing the risk of complications

  • Communicating with healthcare providers

  • Effective symptom management and coping skills

  • Goal setting, decision making, and problem solving
